Early Adopters of the Resource Connection Network
Pilot Group: Petaluma Region
Our initial pilot cohort began in 2022 with voluntary organizations that self-selected to begin using the Resource Connection Network to implement closed-loop referrals into their existing referral workflows. These first adopters include:
Petaluma Health Center: exploring use of RCN through their Comprehensive Perinatal Services Program (CPSP) and Patient Navigators
Sonoma County Office of Education: Using RCN through their Behavioral Health Department Staff
Sonoma County Human Services Department: using RCN through the Trauma-informed Public Health Nurse Perinatal Services Home Visiting Program
Data Exchange Framework Grant Collaborative Partners
On July 5, 2022, CalHHS released the Data Exchange Framework, Data Sharing Agreement, and an initial set of Policies and Procedures that govern the exchange of health and social services information among health care and social services organizations and government agencies. The DxF TA grant supports the development of infrastructure to achieve DxF TA requirements.
We are working with the following agencies, that have signed the CalHHS Data Exchange Framework Data Sharing Agreement, to improve referrals between CBOs and multi-sector partners and to help address barriers that impact their clients’ ability to access support:
Child Parent Institute: Using RCN through their Resource Navigation program and Home Visiting Program.
Family Justice Center: Using RCN making and receiving referrals through their Trauma Recovery Center so victims of domestic violence can receive whole person care.
La Luz Center: La Luz’ Advocate team will be helping clients with unmeet basic needs connect to additional services and support.
Petaluma People Services: Petaluma People Services is connecting their Senior Services, Youth Programs, that specifically provide Counseling and Housing support, to the Resource Connection Network and accepting and sending out referrals.
Capacity and Infrastructure Transition, Expansion, and Development (CITED) PATH Partners
Sonoma Connect is working with CITED funds to implement closed-loop referral infrastructure to support implementation of CalAIM Enhanced Care Management (ECM) and Community Supports (CS) services. Our CITED partners represent Community Based Organizations that are providing ECM/CS services including resource navigation, mental health services, medically tailored meals, housing support, and more.
These partners are utilizing the Resource Connection Network to receive and send referrals for clients that qualify for ECM, CS, and other social services. We are working with the following agencies:
Sonoma Community Action Network (Sonoma CAN): Sonoma CAN staff will use RCN to support conducting Enhanced Care Management services and to receive referrals to their HEAL program, which provides an array of services, including parent education and 0-5 school readiness. Sonoma CAN will leverage RCN infrastructure for their Housing Navigation, Housing Tenancy, and Rental Assistance Community Support services.
Hanna Center: Hanna Center is leveraging RCN infrastructure for their Community Health Hub to receive Enhanced Care Management referrals for families seeking mental health support in Sonoma Valley and to refer them to additional services.
Food For Thought: Food For Thought is leveraging RCN infrastructure to receive Community Supports referrals for individuals needing Medically Tailored Meals.
Community Support Network: Community Support Network is leveraging RCN infrastructure to receive Enhanced Care Management referrals and Housing Navigation, Housing Tenancy, and Rental Assistance Community Supports services.
La Familia Sana: La Familia Sana is leveraging RCN infrastructure to receive referrals to their Housing Navigation, Housing Transition, and Housing Deposits services.
Road to Resilience Partners
This grant, funded by the Office of Child Abuse Prevention, is led by First 5 and Child Parent Institute, and is designed to help parents with a history of Substance Use Disorder access Home Visiting Substance Use Disorder and Mental Health Support. Sonoma Connect will be working with the collaborative partners that provide these services to integrate into our growing list of implementors of the Resource Connection Network

This collaborative grant, funded by the Administration for Children and Families, is led by Sonoma CAN, and provides an opportunity to identify and collectively address gaps and build a seamless Early Childhood-Child Welfare system of care for foster children ages 0-5.

ACE’s Aware UCAAN Birth Equity Partners
This grant, led by First 5, and Sonoma Connect as a subcontractor, is focused on increasing utilization of MediCal’s Enhanced Care Management CHW services through CalAIM for the Birth Equity Population of Focus. It aims to increase infrastructure to address disparities in birth health outcomes for racial and ethnic groups with high maternal morbidity and mortality rates.
The Birth Equity Population of Focus is designed to support Black, American Indian/Alaskan Native, and Asian American demographics.
